| Field | Description |

| CIS Code | This field displays the valid CIS code. |
| CIS Type | This field displays the valid CIS type. |
| Certificate/Registration | This field displays the certificate or registration number. |
| Start Date | This field displays the start date. |
| Expiry Date | This field displays the expiry date. |
| National Insurance No | This field displays the alphanumeric national insurance number. |
| Authorized User Name | This field displays the name of the authorized user. |
| Trading As Name | This field displays the concern's Trading As name. |
| Concern Name | This field displays the name of the concern. The name must be identical to the name on the certificate or registration. |
| Bank Name | This field displays the bank name of the CIS vendor. |
| Bank Address | This field displays the bank address of the CIS vendor. |
